I don't know if I'm the first to come up with idea, but it goes like this.
As far as I can remember there are no doctors in the Pokemon world. There are only nurses. Only one nurse Joy is needed to fully run a Pokemon center. This seems very odd at first, but I think this may be the future of modern medical service in real life. Technology is taking the place of doctors these days. Robots can diagnose and perform surgeries now. In the future, all a hospital would need is a kind face to welcome patients and help them relax while getting healed. This has already happened in the Pokemon world. Technology has developed to the point that Pokemon can be healed just by putting their Pokeballs in a machine behind the front desk or keeping them in incubators for a few days. That's it. All doctors have lost their jobs
Okay, we don't know what the case is for human hospitals in the Pokemon world, since we've never properly seen one. But I think it would most likely be the same.
